The Braden Scale is a clinically validated tool to assess the risk of developing pressure sores (bed sores). Select the option that best describes the current condition for each category. Higher scores indicate lower risk.

Understanding the Braden Scale
Score Range: 6-23 points. Lower scores indicate higher risk of developing pressure sores.
Risk Categories:
- • 19-23: Low Risk
- • 15-18: Mild Risk
- • 13-14: Moderate Risk
- • 10-12: High Risk
- • 9 or below: Severe Risk
Important: This assessment should be repeated regularly (weekly for low risk, daily for high risk). Any change in condition warrants immediate reassessment.
