Understanding the Braden Scale

Score Range: 6-23 points. Lower scores indicate higher risk of developing pressure sores.

Risk Categories:

  • • 19-23: Low Risk
  • • 15-18: Mild Risk
  • • 13-14: Moderate Risk
  • • 10-12: High Risk
  • • 9 or below: Severe Risk

Important: This assessment should be repeated regularly (weekly for low risk, daily for high risk). Any change in condition warrants immediate reassessment.
